This form is to be completed by organisations being asked to participate in the production of video, audio or photographic work for Ritelite (Systems) Ltd.

Gives permission to Ritelite (Systems) Ltd to take photographs and / or video of its members and grants full rights to use the images resulting from the photography / video filming, and any reproductions or adaptations of the images for publicity and marketing purposes.

This might include (but is not limited to), marketing and educational material, print, digital and electronic, including use on social media (for example Instagram or Facebook), published by Ritelite, partners and/or agents authorised by Ritelite (Systems) Ltd

I understand that:

editing may be required by Ritelite, partners and/or agents authorised by Ritelite for quality or technical purposes
edited subsections may be used in other Ritelite materials for purposes stated at the top of the form
the image(s) and/or recording(s) will be stored securely in appropriate file formats on servers belonging to Ritelite, partners and/or its authorised agents.

In signing this form, you confirm that the club / organisation named above holds any relevant permissions required between itself and its players.
